The Professional Regulation Commission is now accepting applications for the June 03 and 04, 2024 Occupational Therapists Licensure Examination from March 01 to May 06, 2024.

Degree/Course Requirement: Bachelor of Science in Occupational Therapy
1. Duly accomplished online system generated Application Form from online.prc.gov.ph with indicated appointment date, reference number of P900.00 Examination Fee, and affixed documentary stamp;
2. Original and photocopy of Philippine Statistic Authority (PSA) Birth Certificate (if Birth Certificate issued by PSA is not readable, attach Birth Certificate issued by the Local Civil Registry);
3. Original and photocopy of PSA Marriage Contract (for married female applicants only);
4. Original and photocopy of Transcript of Records with scanned picture indicating the Special Order (S.O.) number and Remarks "For Board Examination Purposes";
5. Certificate of Authentication and Validation (CAV) from the CHED in case the school is exempted from the issuance of an S.O.;
6. Certified true copy of certificate of practicum training, practice placement, or work experience, specifying the institution where the placement training was undertaken, and the activities or occupational therapy services, and number of hours rendered, under the auspices or supervision of a licensed Occupational Therapist. This certification must be signed and issued by the same authority or superior, and duly notarized;
7. One (1) colored passport size pictures with white background and complete name tag; and
8. Other documents that may be required by the Board.
* The submission of the Birth and Marriage Certificates shall be subject to the provisions of RA No. 11909 or the “Permanent Validity of the Certificates of Live Birth, Death and Marriage Certificates”

Test results are final and there is NO RE-CHECKING OR VIEWING OF TEST PAPERS pursuant to PRC Resolution No. 2004-223.

2. Read the WEBSITE’S USER GUIDE first
3. Click SIGN IN for those with EXISTING ACCOUNT
Click REGISTER to CREATE NEW ACCOUNT. Complete the required information correctly then click SIGN IN (Enter registered email address and password to receive the One-Time Password (OTP), copy the OTP to log in your account), complete the required information, save and upload 2x2 ID picture in white background
4. Click SELECT TRANSACTION (Examination) FOR REPEATER EXAMINEES (April 2012 to present): Select EXAMINATION then proceed to payment (No. 6)
5. Select Preferred PRC Regional Office. Click VIEW OTHER DATES if you wish to change the system-generated earliest appointment schedule. NO SAME DAY APPOINTMENT POLICY. TRANSACT ONLY AT THE INDICATED APPOINTMENT PLACE, DATE AND TIME
6. Choose payment channel. You will be re-directed to their website for the processing of payment:
a. MAYA E-WALLET (includes Credit Card, Debit Card, Gcash)
b. LANDBANK or BANCNET (Payment validation within 2-3 working days)
7. Click PRINT DOCUMENT and PRINTER ICON to print the Examination Application Form
8. Proceed to your APPOINTMENT PLACE ON YOUR APPOINTMENT DATE AND TIME to submit the online system-generated form
•For REPEATER EXAM APPLICANTS, check your online account after seven (7) working days if the status is COMPLIED/ISSUED, then PRINT your Notice of Admission for presentation during the scheduled examination in your testing place
