REQUIREMENTS
AGRICULTURIST
1. Original and photocopy of Philippine Statistics Authority (PSA) Birth Certificate.
2. Original and photocopy of PSA Marriage Certificate (for married female)
3. Original and photocopy of Transcript of Records with Special Order (B) for graduates in private schools. If doctorate degree is obtained abroad a certification stating that the college/university has been recognized or authorized to operate.(BS, MS, Ph. D) & Diploma.
4. If employed in the Government, original and certified copy of duly notarized service record.
4.1 If employed in private sector, a certificate under oath issued by the authorized official/officer of the firm, company, association or corporation engaged in the agriculture industry showing that applicant has served for at least five (5) years in a position which requires baccalaureate degree in Agriculture(with cert. of detailed job description)
5. Certification under oath of the authorized official/officer of a private school, college/university showing that the applicant has served at least five (5) years before December 1997 as faculty member/instructor and has taught subjects in Agriculture.
6. Original and updated NBI Clearance (Private Employee)
7. Original and updated Ombudsman Clearance and NBI Clearance (government employees)
8. Three (3) Certificate of Good Moral Character (barangay, church, school or employer)

FISHERIES TECHNOLOGIST
1. Original and photocopy of PSA Birth Certificate.
2. Original and photocopy of PSA Marriage Certificate (for married female)
3. Original and photocopy of Transcript of Records with Special Order (B) for graduates in private schools. If doctorate degree is obtained abroad a certification stating that the college/university has been recognized or authorized to operate.(BS, MS, Ph. D) & Diploma.
4. Certified copy of report of rating issued by the Civil Service Commission showing that applicant has passed a Civil Service examination in Fisheries or the Career Service Professional Examination.
5. If employed in private sector, a certificate under oath issued by the authorized official/officer of the firm, company, association or corporation engaged in the fishery industry showing that applicant has served for at least five (5) years in a position which requires baccalaureate degree in Fishery (with cert. of detailed job description)
6. Certification under oath of the authorized official/officer of a private school, college/university showing that the applicant has served at least five (5) years before February 1998 as faculty member/instructor and has taught subjects in Fishery.
6. Original and updated NBI Clearance (Private & Government Employee) Original and updated Ombudsman Clearance and NBI Clearance (government employees)
7. Three (3) Certificate of Good Moral Character (barangay, church, school or employer)
Cut-off date: February 1998
REAL ESTATE SALESPERSON
For Real Estate Salesperson with no DTI / HLURB Registration
1. Original and Photocopy of PSA Certificate of Live Birth / Birth Certificate
2. Original and Photocopy of PSA Marriage Certificate / Contract (For married female only)
3. Original and Notarized Certificate of Educational Attainment on the Completion of at least two (2) years of College
4. Original NBI Clearance (Valid)
5. Original and Notarized Certification of eight (8) credit units on Real Estate Brokerage.
6. Photocopy of Broker’s PRC License with 3 signatures
PROFESSIONAL TEACHER
1. Certified true copy of Report of Rating (Category A) / Guro Certificate (Category B), or in the absence thereof, original Certification of Rating/Eligibility (for both categories) issued by the Civil Service Commission (CSC)
2. Original and photocopy of Certificate of Live Birth issued by the Philippine Statistics Authority (PSA) in security paper, with the following entries clearly printed: Name, Civil Registry Number, Date of Birth, Citizenship, Gender, and Name of Parents.
3. Original and photocopy of Marriage Certificate issued by PSA in security paper (for married female applicants)
IMPORTANT: For NON-AVAILABILITY OF RECORDS – Negative Certification issued by PSA and Certificate of Live Birth/Marriage from the Local Civil Registrar (LCR) duly authenticated by authorized PSA Statistician. For Senior Citizen, negative certification issued by PSA or Passport.
4. Original and photocopy of the Transcript of Records for degree earned
5. Three (3) recent identical copies of passport size pictures in white background with complete name tag
6. Current Community Tax Certificate (Cedula)
