REQUIREMENTS
- Valid Professional Identification Card (PIC); If expired, for renewal of PIC; Authorization Letter of the examinee/professional and any valid proof of identity if requested or claimed through a representative.
- Original and photocopy of PRC documents to be authenticated
- Two (2) pieces of documentary stamps per copy
- Duly accomplished Request Form and any valid identification card (for examinees who failed)
- Other requirements stated above pursuant to Office Memorandum No. 2004-07 s. 2004
- Php75.00 Certification Fee per copy

1. Secure Request Form from the Officer-of-the-Day at the Public Assistance and
Complaints Desk (PACD).

- For Certificate of Failure and Board Rating - Valid Identification Card
- For Authentication - Updated PIC, Original and photocopy of Certification of Passing or Failure or Board Ratings to be authenticated
2. Present Request Form and other required documents at Counter 4 (Cashier) and
pay for certification/authentication fees (P75.00 per document).

1. Present Order of Payment and Valid PRC ID for checking at the Public Assistance and
Complaints Desk (PACD)
2. Submit all documents at Counter 2 for processing and wait for the release at the same
counter.
DEPARTMENT OF FOREIGN AFFAIRS (DFA) APOSTILLE
1. Have your PRC Document/s for DFA Apostille authenticated.
2. Submit authenticated copies of PRC documents including the Envelope for
DFA Apostille at Counter 2.
PRC Documents for Apostille:
a. Certificate of Passing (CP)
b. Certificate of Board Ratings (BR)
c. Official Report of Ratings (ORR)
d. Certificate of Good Standing (CGS) - must indicate an unexpired validity date
f. Professional Identification Card (PIC) - must be valid and with signature at the back
g. Certificate of Registration (COR) / Board Certificate (BC)
3. Receive the sealed PRC envelope and bring it personally to DFA for Apostille.
Registered professionals in good standing and with no pending legal/administrative case (pursuant to Regional Office Order No. BAG-2012-25).

- Payment of certification/authentication fee – Php75.00 per copy; and
- Two (2) piece documentary stamp per copy.

NEW ACCOUNT, click REGISTER (enter the required information correctly), SIGN
IN (Enter registered email address and password to receive the One-Time
Password (OTP), copy the OTP to log in your account), complete the required
information, save and upload 2x2 ID picture in white background
4. Select Transaction (Certifications or Authentications)
5. Select Preferred PRC Regional Office. Click RESCHEDULE? if you wish to change the system-generated schedule. NO SAME- DAY APPOINTMENT POLICY. TRANSACT ONLY AT THE INDICATED APPOINTMENT PLACE
6. Choose payment option:
A. PAYMAYA
USING CREDIT OR DEBIT ATM CARD or PAYMAYA WALLET or GCASH.
B. LANDBANK
or BANCNET (Payment
validation within 2-3 working days)
7. Click PRINT DOCUMENT and PRINTER ICON to print for:
Certification → Order of Payment
Authentication → Action Sheet
8. Proceed to your APPOINTMENT PLACE ON YOUR
APPOINTMENT DATE to submit form/requirements (Note: Advance
Transaction is not Allowed).
