A. Correction of entries:
1. Duly accomplished and notarized Petition Form for correction of entries/data
2. Affidavit of Discrepancy (1 original)
3. Philippine Statistics Authority (PSA) copies of Certificate of Birth
and/or Certificate
of Marriage (1 original)
4. Photocopy of Professional Identification Card (PIC)
5. One (1) documentary stamp
B. Change of status:
I. Due to Marriage
1. Duly accomplished and notarized Petition Form for Change of Status of Registered Name Due to Marriage
2. PSA copy of Certificate of Marriage (1

3. Photocopy of PIC
4. One (1) documentary stamp
II. Due to Reversion/Remarriage
2. PSA copy of Certificate of Live Birth (1 original)
3. PSA copy of Certificate of Marriage with annotations in case of
nullity/annulment or Certificate of Death in case of death of spouse (1 original)
4. PSA copy of Certificate of Marriage of subsequent marriage, if applicable
6. Photocopy of PIC
7. One (1) documentary stamp
of Marriage (1 original)
4. Photocopy of Professional Identification Card (PIC)

Other requirements:
1. Certificate of Marriage from the Local Civil Registrar,
if the entries in the Certificate of

2. PSA Certificate of Live Birth, if there is/are erroneous
entry/ies in the PSA Certificate of
Marriage (1

3. Certificate of Live Birth from the Local Civil Registrar, if
there is/are entry/ies in the Certificate of
Marriage from PSA are unreadable (1

4. Affidavit of discrepancy, if there is/are discrepancy/ies in the
documents submitted (1 original)
5. For representatives:- Valid government-issued ID and Special Power of Attorney executed by the petitioner if representative is not a holder of valid PRC ID
- Valid PRC ID and letter of authority from the petitioner if representative is a registered professional
STEPS AND PROCEDURE
Step 1:
Present notarized Petition Form and other basic requirements at the Regulation Division for evaluation and verification of examination and registration records
Step 2:
Pay the prescribed fee of PhP225.00 at the Cashier (Counter 4)
Step 3:
Submit the Petition Form with the attached requirements at the Regulation Division.
NOTE:
Client is advised to verify through www.prc.gov.ph > Other eServices > Verification of Licenses > Verification of License (By License No) after seven (7) working days if amendment has already been reflected in the system, after which he/she may already file for the renewal or duplicate copy of his/her Professional Identification Card (PIC) at https://online.prc.gov.ph using updated entries.
- Valid government-issued ID and Special Power of Attorney executed by the petitioner if representative is not a holder of valid PRC ID
