The Professional Regulation Commission Baguio Regional Office is now accepting
applications for the March 25, 2018 Board Licensure Examination for Teachers in
Baguio City, Rosales in Pangasinan, and other PRC regional offices.
Requirements for new/first-time and repeater applicants are:
- Original and photocopy of Transcript of Records with scanned picture and remarks, “For Board Examination Purposes”;
- Original and photocopy of PSA/NSO-issued Birth Certificate issued in security paper. Submit a copy of Birth Certificate from Local Civil Registrar if PSA/NSO copy is not legible;
- Original and photocopy of PSA/NSO-issued Marriage Certificate in security paper. Submit a copy from Local Civil Registrar if PSA/NSO copy is not legible (for married female applicants);
- One (1) piece RECENT passport-sized colored picture in plain white background with complete name tag (Last Name, First Name, Middle Name);
- Recent Community Tax Certificate (Cedula);
- One (1) piece documentary stamp with face value of Php15.00;
- One (1) piece window envelope with postage stamp face value of Php16.00.
Deadline of application is on January 19, 2018.
To save on time and expenses, applicants may transact with the PRC personnel during the scheduled mobile outreach program posted at www.prcbaguio.blogspot.com. Applicants MUST APPLY ONLINE at www.prc.gov.ph or www.prc-online.com through the Online Application, pay the prescribed fee at any authorized payment centers (7- Eleven, Western Union, Bayad Center, Multipay, ECPay, etc.) and print the Application Form before proceeding to the Baguio Regional Office or in mobile outreach program venues. APPLICANTS MUST APPLY PERSONALLY AND STRICTLY FOLLOW THE
APPOINTMENT SCHEDULE. No extension of deadline.
