INITIAL REGISTRATION

WHO CAN AVAIL INITIAL REGISTRATION
  • Those who passed the Licensure Examinations; and/or
  • Those whose applications for Registration without Examination have been approved by the Board through Board Resolutions.
REQUIREMENTS FOR INITIAL REGISTRATION 

  • Duly accomplished Oath Form/ Panunumpa ng Propesyonal;
  • Two (2) copies passport size white background, complete name tag (Middle Name must be spelled out) and in decent attire;
  • Current Community Tax Certificate (Cedula);
  • Payment of registration Fees – P1,050.00 for baccalaureate; P870.00 for non-baccalaureate; and
  • One (1) piece documentary stamp.

REQUIREMENTS FOR REGISTRATION OF PASSERS ABROAD 

  • Oath Form (Panunumpa ng Propesyonal) - downloaded including personal information and picture at www.prc.gov.ph  Must be notarized at the Consulate Office in the country of residence
  • Note: Representative has to secure from PRC blank Registry Sheet to be sent to the registrant for filling out and must be submitted to this office by the same representative together with all the requirements.
  • Special Power  of Attorney from the Consul/Notary Public abroad;
  • Oath Form filled-out by the applicant and notarized by the Consul/Notary Public abroad;
  • Acknowledgment from the Consul;
  • Photocopy of the passport pages showing personal data and date of entry abroad; and
  • Registration fee – P1,050.00

STEPS AND PROCEDURES

  1. Secure downloaded Oath Form from Counter 1, Records Section at the second floor of the building; fill-out form and submit requirements to Processor at Counter 12 or 13.
  2. Registration Officer verifies name of registrant, enters assessment of fees and advises the registrant on the amount to be paid.
  3. Give Payment.
  4. Staff indicates the amount received on the Oath form and gives to the Cashier.
  5. Cashier receives the Oath form/payment, issues Official Receipt(O.R.) and gives back the form  with the O.R. (and change, if any) to the Registration Officer.
  6. Registration Officer prints the Registry Sheet.
  7. ffix three (3) specimen signatures and thumb mark on the Registry Sheet.
  8. Claim stub from Registration officer and change, if any.
  9. Check PIC availability at www.prcbaguio.blogspot.com (PICAV).

OATHTAKING OF PROFESSIONALS

All Registrants are required to take their oath of profession through -

  • Mass Oathtaking conducted by the concerned Board; 
  • Registrants who failed to join the mass oathtaking ceremony may attend the special oathtaking for free every Thursday at the Baguio Regional Office at 11:00 o’clock in the morning and 2:00 o’clock in the afternoon.
  • Register at Window 12 for attendance.
  • Please come in semi-formal attire. Health and allied health professionals must come in gala attire.
