The
Professional Regulation Commission (PRC) Baguio
Regional Office is now accepting re-taker applications through the Online
Application System (OAS) for the June-July 2012 NLE. First-timers are advised
to apply through OAS starting April 21.
Schedule
Re-takers
can apply through OAS from April 12 to April 19 only. On April 20 (deadline), re-taker
applicants have to apply at PRC Baguio.
The
OAS is limited initially to Baguio, Cabanatuan, Dagupan and Laoag examinees. Ilocos
Region Satellite Office in San Fernando City, La Union will process manual
applications only.
Steps
Re-taker
applicants can file online on any computer with internet, through the following steps:
1. Visit
www.prcbaguio.blogspot.com
for instructions and CLICK the Online Application System (OAS) link found HERE.
2.
Encode
examination-related information thru the Data Entry Form.
3.
Click
SAVE and OK to submit your examination request.
4.
Wait
for Appointment Notice through email for your appointment (date and time you
will go to PRC Baguio). Do not go to PRC Baguio without an Appointment Notice.
5.
On
your appointment date/time:
5.1 Proceed to CSCMPC to
pay for documentary stamps and postage-stamped PRC envelope;
5.2 Proceed to Window A (Processor)
and submit the following:
a.
One
(1) passport size picture for scanning;
b.
Notice
of Admission (NOA) or Official Report of Ratings or PRC Official Receipt of
previous NLE;
c.
In
case of change of surname due to marriage by female married applicant, present original
and photocopy of NSO Marriage Certificate. For change of Name or Date of Birth
due to Court Decision, submit original and photocopy of annotated NSO Birth
Certificate (and Annotated NSO Marriage Certificate for female married applicant)
or certified copy of Finality and Court Decision.
5.3 Receive your Nurse
Application Form (NAF) from the Processor;
5.4 Proceed to Window B
(Cashier) and pay Application Fee of P900.00;
5.5 Proceed to Window C
(Issuing Officer) and submit NAF with your requirements and postage-stamped PRC
envelope;
5.6 Receive your Notice
of Admission (NOA) and Examinee’s Guide.
5.7 Download and print Examination

and Nueva Ecija re-takers MUST apply through OAS before their applications can
be processed during the scheduled Agew na Propesyonal on the
