REQUIREMENTS FOR RENEWAL OF PROFESSIONAL IDENTIFICATION CARD


WHO CAN AVAIL
For professionals whose Professional Identification Card (PIC) already expired, or about to expire within a year.


REQUIREMENTS

1. Duly accomplished Form. (Download Renewal Form HERE)
2. Photocopy of latest Professional Identification Card (PIC). In case of loss of unexpired PIC, submit duly notarized Affidavit of Loss.
3. Two (2) copies of RECENT colored passport-size pictures in white background with complete name tag (Last Name, First Name Middle Name) (1 pc only for Teachers).
4. Payment of renewal fee. (Click HERE to view Assessment of Fees Table)


ADDITIONAL REQUIREMENT AS PRESCRIBED BY RESPECTIVE BOARDS
a. Architects - Photocopy of valid UAP Membership ID/Certification of Good Standing
b. Customs Broker Certification of Good Standing from CCBI
c. Electronics Engineer/Prof. Electronics Engineer - IECEP Certification of Good Standing
d. Geodetic Engineer - Photocopy of valid G.E.P. Membership ID/Certification of Good Standing.
e. Guidance and Counseling - Valid Certificate of PGCA Membership and annual payment receipt.
f. Mechanical Engineer/Prof. Mechanical Engineer - Certification of Good Standing from Phil. Society of Mechanical Engineers holding office at Don Lorenzo Bldg., P. Paredes Street, Sampaloc, Manila and its duly accredited chapters.
g. Prof. Agricultural Engineer - Photocopy of PSAE Membership ID/ Certification of Good Standing
h. Prof. Electrical Engineer/Registered Electrical Engineer/Registered Master Electrician - IIEE Membership ID/ Certification of Good Standing

ADDITIONAL REQUIREMENTS FOR FEMALE MARRIED PROFESSIONAL WITH REQUEST FOR CHANGE OF NAME(CON) and CHANGE OF STATUS(COS) DUE TO MARRIAGE

  1. Duly accomplished Petition Form for change of name/status;
  2. Original copy of Marriage Certificate issued by NSO in security paper;
  3. One(1) piece documentary stamp; and
  4. Payment of petition fee - Php 225.00

PROCEDURE


Step 1 - Get Renewal Form from the Public Assistance and Complaints Desk or download Form at www.prcbaguio.blogspot.com (and Petition Form for married female professional requesting for CON/COS).


Step 2 - Accomplish renewal/petition form and paste passport-size colored picture in white background with complete name tag.


Step 3 - Proceed to Renewal Section (Window 7 or 8); present your accomplished renewal/petition form and Professional Identification Card for assessment of fees, verification and evaluation of requirements.


Step 4 - After verification, the professional is advised to proceed to Cashier (Window 5 or 6) for payment of prescribed fees;


Step 5 - Proceed to Renewal Section (Window 7 or 8) and submit all documents including Official Receipt.

Step 6 - Receive your Claim Slip/Stub from the Registration Officer.

INSTRUCTIONS ON HOW TO CLAIM:

1. If your name or license number does not appear in the list, your Professional Identification Card (PIC) may still be with PRC Central Office for processing. You may also verify with the Regulations Division at prcbaguio.regulation@gmail.com

2. If ID is already available, you can claim the same personally at Window 9, Monday to Friday (7 am to 6 pm) or through a representative who must present:

  • Authorization letter
  • Claim stub
  • Your valid identification card with picture
  • Representative’s valid identification card with picture.

PETITION FOR CORRECTION OF ENTRIES/DATA  (NAME / DATE OF BIRTH)

Registered professionals who need to correct entries/data in the records of the Board and the Commission in order to conform to the true and correct spelling of Given Name/Middle Name/Surname and the correct Date of Birth.

REQUIREMENTS

  • Photocopy of unexpired/current PRC ID;
  • Original copy of Certificate of Live Birth from NSO in security paper indicating the Annotations and/or Supplemental Report;
  • Original copy of Marriage Certificate from NSO in security paper indicating the Annotations and/or Supplemental Report (for female married professional);
  • Personal Affidavit of Discrepancy duly notarized;
  • Three (3) copies of recent colored passport-size pictures in white background and full name tag (Last Name, First Name Middle Name);
  • Payment for CON/CODB fees in the amount of Php225.00; and
  • One (1) piece documentary stamp.

PROCEDURE

  1. Get Petition Form from the Public Assistance and Complaints Desk;
