CONDUCT OF THE DECEMBER 2021 PHYSICAL THERAPISTS AND OCCUPATIONAL THERAPISTS LICENSURE EXAMINATIONS

The Professional Regulation Commission (PRC) announces the conduct of the December 5 and 6, 2021 Physical Therapists Licensure Examination (PTLE) and Occupational Therapists Licensure Examination (OTLE) in Baguio City to 162 and 9 examinees, respectively. The testing venue is at the University of Baguio, General Luna Road.

All examinees are required to accomplish the entry record sent to their email addresses or posted at www.facebook.com/prc.cordillera for monitoring and contract tracing purposes.

Examinees coming OUTSIDE of Baguio, La Trinidad, Itogon, Sablan, Tuba and Tublay shall strictly adhere to the latest entry protocols of Baguio City as posted www.facebook.com/pio.baguio such as the presentation of the following at the Baguio City Central Triage:

1. If fully vaccinated in a legitimate facility outside of Baguio:
a. QR Code issued through a duly registered Health Declaration Form at https://hdf.baguio.gov.ph;
b. COVID-19 domestic Vaccination Certificate issued by a legitimate vaccinating establishment or International Certificate of Vaccination issued by the Bureau of Quarantine; and
c. Notice of Admission (NOA) or Room Assignment as proof of indispensable transaction.

2. If not fully vaccinated:
a. QR Code issued through a duly registered Health Declaration Form at https://hdf.baguio.gov.ph;
b. NOA or Room Assignment as proof of indispensable transaction; and
c. Negative results RT-PCR test conducted within seventy-two (72) hours prior to entry into Baguio.

At the TESTING VENUE, examinees shall bring/submit the following:

1. NOA;
2. Ballpens and pencils;
3. Any of the following:

a. Photocopy of the complete Vaccination Card if fully vaccinated (An individual is considered FULLY VACCINATED FOR COVID-19 after a period of at least two (2) weeks from having received the second dose of a 2-dose series; or after a period of two (2) weeks from having received a single-dose vaccine);
b. Negative result of the RT-PCR test (NOT Antigen test), date of collection shall be within the period November 30 to December 4;
c. Certificate of Mandatory Quarantine for 14 days that shall end within the period November 30 to December 4, or its equivalent signed by any of the following: Any government or private physician, Municipal Health Officer, Provincial Health Officer, City Health Officer, Barangay Officer visiting the examinee’s residence attesting that he/she has been in quarantine for 14 days

4. Duly accomplished Informed Consent Form and Health Declaration Checklist (Annexes A and B of Memorandum Order No. 68 downloadable at www.prc.gov.ph);
5. N95 or KN95 face mask and face shield to be worn at all times;
6. 70% ethyl alcohol for regular hand disinfection; and
7. Pre-packed meal/snacks and drinks.

Refer to the program of examination for other allowed and prohibited items.

Examinees shall submit the Post Examination Health Surveillance Form (Annex C of Memorandum Order No. 68) to car.examination@prc.gov.ph 15 days after the examination or on December 21, 2021.

An examinee shall be allowed to defer taking the licensure examination provided that the following documents are submitted on or before November 20, 2021:

1. Written request duly signed by examinee; and
2. Photocopy or electronic copy of the NOA.

Transfer to another testing center SHALL NO LONGER BE ALLOWED.

In the event of failure to take the November 2021 PTLE and OTLE, examinees may submit a written request with the required supporting documents on or before December 13, 2021 and shall be allowed to take the June 2022 PTLE and OTLE subject to the following valid reasons:

a. For health-related reasons – shall be supported with a medical certificate;
b. For quarantine-related travel restrictions – shall be supported with an official certificate of border/travel restrictions issued by the concerned local government unit.

The written request and supporting documents shall be submitted through email at car.application@prc.gov.ph or car@prc.gov.ph. Said examinees are advised to keep their Notice of Admission for use during the next examination.

Examinees and examination personnel are reminded to strictly observe the minimum health standards set by the Department of Health and to comply with the provisions of PRC Memorandum Order No. 2020-68 dated November 06, 2020 re: Updated Standard Guidelines on the Strict Health Protocols to be Observed in the Conduct of Licensure Examinations During the COVID-19 pandemic for everyone’s safety, and to immediately go home after the conduct of the examination. With Baguio’s weather condition, examinees and examination personnel are advised to use additional layer of clothes, bring rain gears, and flashlights in case of power outage at the testing venue.
