APPLICATION FOR THE LICENSURE EXAMINATION FOR TEACHERS

The Professional Regulation Commission Baguio Regional Office is now accepting applications for the September 30, 2018 Licensure Examination for Teachers until July 20, 2018.

Requirements for new/first-time and repeater applicants are:

1.    Original and photocopy of Transcript of Records with scanned picture and remarks, “For Board Examination Purposes”;

2.    Original and photocopy of PSA/NSO-issued Birth Certificate issued in security paper.  Submit a copy of Birth Certificate from Local Civil Registrar if PSA/NSO copy is not legible;

3.    Original and photocopy of PSA/NSO-issued Marriage Certificate in security paper.  Submit a copy from Local Civil Registrar if PSA/NSO copy is  not legible (for married female applicants);

4.    One (1) piece RECENT passport-sized colored picture in plain white background with complete name tag (Last Name, First Name, Middle Name);

5.    Any valid government issued ID as competent proof of identity; and

6.    Documentary stamps with face value of Php30.00;

To save on time and expenses, applicants may transact with the PRC personnel during the scheduled mobile outreach program posted at www.prcbaguio.blogspot.com. Applicants MUST APPLY ONLINE at www.prc-online.com through the Online Exam Application, pay the prescribed fee at any authorized payment centers (7-Eleven, Multipay, ECPay, Bayad Centers (Western Union, Villarica Pawnshop, etc.) and print the Application Form before proceeding to the Baguio Regional Office, Robinsons Place Ilocos Service Center, Robinsons Place Pangasinan Service Center or at any mobile outreach program venues in CAR and Region I. For those filing during the scheduled mobile outreach program, please select among the listed PREFERRED REGIONAL OFFICE (e.g. Lagawe, Vigan City, Candon City, Alaminos City, Urdaneta City, Rosales) as YOUR TRANSACTION WILL BE PROCESSED ONLY AT YOUR INDICATED PLACE OF TRANSACTION AND ON THE DATE IDENTIFIED.
