Requirements
for new/first-time and repeater applicants are:
1.
Original and photocopy of Transcript of
Records with scanned picture and with remarks, “For Board Examination
Purposes”;
2.
Original and photocopy of PSA/NSO-issued
Birth Certificate issued in security paper.
Submit a copy of Birth Certificate from Local Civil Registrar if PSA/NSO
copy is not readable;
3.
Original and photocopy of PSA/NSO-issued
Marriage Certificate in security paper.
Submit a copy from Local Civil Registrar if PSA/NSO copy is not readable
(for married female applicants);
4.
One (1) piece RECENT passport-size colored
picture in plain white background with complete name tag (Last Name, First
Name, Middle Name);
5.
Any government-issued identification (passport,
driver’s license, PRC, UMID, digitized BIR, SSS, Postal, NBI Clearance, Police
Clearance, voter’s ID, Philhealth card);
6.
One (1) set documentary stamp with face value
of Php30.00;
7.
One (1) piece window envelope with face value
of Php15.00 postage stamp for Certified Public Accountants and Environmental Planners
applicants; and
8.
Notice of Admission (NOA) or official receipt
from previous examination, or Official Report of Ratings of previous
examination as additional requirement for repeaters.
Additional
requirements for Civil Engineers exam applicants:
1.
Valid NBI Clearance
2.
Two (2) Certificates of Good Moral Character
from either of the following: Barangay, Church/Parish Priest, Dean of School or
Employer
Additional
requirements for Certified Public Accountants exam applicants:
1.
Valid NBI Clearance
2.
After two (2) failures, Refresher Course
Additional
requirement/s for Nurses exam applicants:
1.
For Nursing graduates under CHED Memorandum
Order No. 14 Series of 2009, to submit Notarized Certificate of Undertaking
2.
For Nursing graduates under CHED
Memorandum Order No. 30 s. 2001, to submit:

Original copy of the Summary of
Related Learning Experience
b.
Original copy of the Operating
Room Cases
c.
Original copy of the Delivery
Cases
Additional
requirements for Environmental Planners exam applicants:
1.
Valid NBI Clearance
2.
One (1) Certificate of Good Moral Character
from either of the following: Barangay, Church/Parish Priest, Dean of School or
Employer
3.
OJT Certificate or In-service Training signed
by the applicant’s immediate supervisor
Additional
requirements for Criminologists exam applicants:
1.
Valid NBI Clearance
2.
RTC Clearance
3.
Three (3) Certificates of Good Moral
Character from either of the following: Barangay, Church/Parish Priest, Dean of
School or Employer
