The oath-taking ceremonies of the successful examinees will be held at the Easter College Gym on December 12, 2011 at 9:00 AM.
Initial registration will start on November 28, 2011 at Baguio Regional Office. For initial registration outside of Baguio, see schedule of Agew / Aldaw / Araw ng Propesyonal posted at www.prcbaguio.blogspot.com. Successful examinees should personally register and sign in the Roster of Registered Professionals. Requirements for the initial registration are:
o Duly accomplished Oath Form or Panunumpa ng Propesyonal;
o 2011 Community Tax Certificate (Cedula);
o Two (2) pieces passport size pictures (colored with white background and complete name tag);
o One (1) piece 1”x1” picture (colored with white background and complete name tag);
o Photocopy of NSO Birth Certificate;
o One (1) Short Brown Envelope;
o One (1) documentary stamp for the Oath Form; and
o Initial Registration Fee of P600.00 and Annual Registration Fee of P450.00 for 2011-2014.
