I. BASIC REQUIREMENTS
A. FIRST TIMERS
DOCUMENTARY REQUIREMENTS
a. Original and photocopy of Transcript of Records (TOR) with Scanned Picture, indicating the Special Order (S.O.) number unless it is not required and with remarks “FOR BOARD EXAMINATION PURPOSES ONLY”;
Note: For All New Schools/Programs, CHED Recognition Certificate is required
b. Original and photocopy of Certificate of Live Birth in NSO security paper (SECPA), photocopy of Birth Certificate from Local Civil Registrar if NSO copy is not legible/readable;
c. Marriage Certificate in NSO security paper for Married Female Applicants, photocopy of Marriage Certificate from Local Civil Registrar if NSO copy is not legible/readable;
d. Two (2) identical passport size (1.72”x1.37”) colored pictures in white background taken within three (3) months with complete name tag (Surname, First Name Middle Name);
e. Community Tax Certificate (CEDULA);
f. Examination Fee of Php900.00 (Baccalaureate Exam) and Php600.00 (Non Baccalaureate Exam).
B. REPEATERS / RETAKERS
DOCUMENTARY REQUIREMENTS
a. Any of the following:
Notice of Admission from previous examination
Official Receipt (O.R.) from previous examination
Official Report of Rating
Transcript of Records (If no copy of Notice of Admission, Official Receipt, or Report of Rating)
b. Marriage Certificate in NSO security paper for Married Female Applicants, photocopy of Marriage Certificate from Local Civil Registrar if NSO copy is not legible/readable;
c. Two (2) identical passport size (1.72”x1.37”) colored pictures in white background taken within three (3) months with complete name tag (Surname, First Name Middle Name);
d. Community Tax Certificate (CEDULA);
e. Examination Fee of Php900.00 (Baccalaureate Exam) and Php600.00 (Non Baccalaureate Exam);
f. Original copy of NBI Clearance (National Bureau of Investigation) for the following Licensure Examinations:

Chemical Engineer
Dentist
Dental Hygienist
Dental Technician
Guidance and Counselling
Psychology
Psychometrician
Real Estate Appraiser
Real Estate Broker
Real Estate Consultant
Respiratory Therapy
Veterinary Medicine
g. For Criminology - Original copy of NBI Clearance (National Bureau of Investigation) or Police Clearance with Court Clearance.
h. For Sanitary Engineering – must submit all the basic requirements and application will be forwarded to the Board (FTB) for approval.
II. ADDITIONAL REQUIREMENTS (FOR THE FOLLOWING BOARD LICENSURE EXAMINATIONS ONLY)
• Certified Public Accountants (CPA)
- Every 2 failures
- Submit refresher course certificate
• Agricultural Engineering
- Every 3 failures
- To rest for 1 year
• Agriculturist
- Every 3 failures
- To submit 6 month refresher course certificate – for approval of the Board (FTB)
• Dentists (new law effective Dec. 2008)
- Every 3 failures
- To submit 6 month refresher course certificate (FTB)
• Dental Hygienist/Dental Technician
- Failed 3 times during the period of 5 years;
- Failed 3 times but were CONDITIONED before 2007;
- Those who graduated 2007 & 2008 and failed 4 or 5 times in the examination; and
- Those who have not taken the examination for the past 5 years
- To submit 6 month refresher course certificate (FTB)
• Fisheries
- Every 3 failures
- To submit 6 month refresher course certificate
• Geodetic Engineering
- Every 3 failures
- To rest for 1 year
• Interior Design
- Every 3 failures
- To rest for 1 year
• Landscape Architect
- Every 3 failures
- To rest for 1 year
• Mechanical Engineering
- Every 3 failures
- To rest for 1 year
• Certified Plant Mechanic (CPM)
- Every 3 failures
- To rest for 1 year
• Medical Technology
- Every 3 failures
- To submit refresher course/training for 12 months (FTB)
• Naval Architecture
- Every 3 failures
- To rest for 1 year
• Optometry
- Every 3 Failures
- To submit refresher course (FTB)
• Pharmacy
- Every 3 failures
- To submit review course certificate (FTB)
• Physical Therapist (PT) and Occupational Therapist (OT)
- Every 3 failures
- To submit refresher course

- First 3 failures only
- To submit refresher course, certificate and attendance, Grades in subjects taken (FTB)
• Sanitary Engineering
- Every 3 failures
- To rest for 1 year
III. ERRORS IN THE BIRTH CERTIFICATE, MARRIAGE CERTIFICATE (for female married applicants) and TRANSCRIPT OF RECORDS
a. Inconsistencies in the usual writing or printing of names such as:
* MA and MARIA
* JR and JUNIOR
* DE LA and DELA
* DE LOS and DELOS
* MARYJANE and MARY JANE
*MARY-JANE and MARY JANE
Submit the Following: Personal Affidavit and Joint Affidavit
b. Discrepancies in vowels/double and single letters in the spelling of name (First Name, Middle Name and Surname)
* RAYMUNDO - REYMUNDO
* BARRAMEDA - BARAMEDA
* MUNDA - MUNDO - MONDO
Submit the Following: Personal Affidavit and Joint Affidavit
c. Wrong Gender, Two (2) Gender or No Gender printed on the Birth Certificate and Marriage Certificate
Submit the Following:
1. Baptismal Certificate
2. Parent’s or Guardian’s Affidavit
3. Certification from the barangay stating that the applicant is really Male or Female
IV. DOCUMENTARY REQUIREMENTS FOR THE FOLLOWING CASES
a. Dual Citizenship / Parents are not Filipino Citizen
Submit the Following:
1. Certificate of Allegiance / Oath of Allegiance from Bureau of Immigration and Deportation (BID)
2. Valid Philippine Passport
3. Voter’s ID / Voters Certificate of Registration
b. Indigenous People
The use of Grandfather’s first name as surname - Submit Certification from the National Commission on Indigenous People (NCIP)
