PROCEDURE IN THE SHIPPING OF CERTIFICATE OF REGISRATION/LUPON/BOARD CERTIFICATE AT THE OPTION OF THE PROFESSIONAL
(Applicable to Examination Passers who had their Registration in Baguio only)
1. Check availability of Certificate of Registration/Lupon/Board Certificate at prcbaguio.blogspot.com. Email and/or text messages were also sent to professionals on the availability of the document. THE SHIPPING SERVICE OFFERED IS AT THE OPTION OF THE PROFESSIONAL.
2. Coordinate with courier service provider in your location (JRS Express/W Express/ABest Express, etc.) that offers 1-pounder two-way/prepaid pouch (applicable in the Philippines only).
3. Insert in the two-way pouch the following requirements:
3.1. Signed request letter with instruction to send via courier
3.2. Two (2) pieces of documentary stamp from BIR
3.3. 2x2 picture in white background
3.4. Photocopy of Professional Identification Card (front portion only) for verification
3.5. Short brown envelope to support the Certificate of Registration
4. Request for two-way pouch may be transacted either personally or through a duly authorized representative. If request is done through a representative, additional requirements are inserted in the two-way pouch:
4.1. Valid ID card of representative
4.2. Photocopy of the Professional’s valid ID card:
4.1.1. Authorization letter (if representative is PRC registered professional)
4.1.2. Special Power of Attorney (if representative is not PRC registered professional)
5. Label the two-way pouch as follows:
